Student Enrollment

Welcome to Lewistown and the Lewistown Public Schools family!  We are excited to partner with you on your child's education.  We are proud of the fact that our schools are some of the best in the state.  They consistently stand out when compared to both state and national scores.  

We are proud to offer a well-rounded curriculum, combined with an abundance of extracurricular offerings, and provide a wealth of opportunities and experiences for youngsters as they make their first steps into the world.

 

Plan for a successful transition ...

When transitioning to a new school, it is always helpful to keep in mind a few things to make that transition easier for your child. 

  • Please contact the Lewistown Public Schools as far in advance of your move as possible and visit with the administrative secretary at the Lincoln Building regarding specific registration considerations.
  • Consider transitioning as near to the beginning of a term as possible.

 

How do I enroll my child in school?

Copies of your child's birth certificate and immunization records must be received by the school BEFORE your child can enroll in Lewistown Public Schools. The immunization records are then reviewed by the School Nurse and you will be notified if any additional shots are required.  

The next step is for a parent, legal guardian, or legal custodian to fill out an enrollment form for each Household. All students in grades K-12 living in that household who plan to attend Lewistown Schools should be listed. Those forms are available in the Enrollment Forms article below.

 

Immunizations & Birth Certificate

Copies of your child's Birth Certificate and Immunization Records must be received by the school before your child may enroll in Lewistown Public Schools. The immunization records are then reviewed by the School Nurse and you will be notified if any additional shots are required.

In order to be admitted, students must show proof of the following immunizations in accordance with Montana Law:

  • diphtheria
  • pertussis
  • tetanus
  • poliomyelitis
  • rubella, mumps, and measles
  • varicella (chicken pox)

Please note: There are new immunization requirement changes that occurred on Oct. 1, 2015 in Montana. Exemptions or conditional attendance procedures are allowed, but must include documentation and forms from the state. Enrollment Forms

A parent, legal guardian, or legal custodian will need to fill out an enrollment forms for each household. All students living in that household who plan to attend Lewistown Schools should be listed. Our downloadable forms are found here.

 

Student Supply Lists

In addition to the Supply Lists on the pages linked below is additional information specific to each school. The schools update them regularly throughout the year so parents are encouraged to check these "Digital Backpacks" which can be found on each schools page within the website. High school supply lists are provided by teachers after school starts.
